The occipito posterior position can complicate labor primarily due to the baby's head being positioned towards the mother’s back, which often leads to a more difficult descent through the birth canal. When the fetus is in this position, the larger part of the head faces the birth canal, making it less favorable for passage. This orientation can result in prolonged labor, increased likelihood of labor augmentation, and a higher chance of the need for instrumental delivery or cesarean section.

In contrast, the occipito anterior position generally allows for easier labor progression as the baby's head tends to align more favorably with the mother's pelvis. The vertex position, which is where the baby is head-down and the top of the head (the vertex) leads the way, usually supports a smoother descent. A transverse lie, while also problematic, typically requires immediate intervention since it presents a sideways position of the fetus, but the occipito posterior position is particularly noted for complications related to descent and labor efficiency.
